Cooling accessories — gimmick or genuinely effective?

Been chewing on this for a while, so I figured I would ask here. Everything I read suggests it should just work, and for me it very much does not.

Quick correction to the above: the defaults are conservative on purpose, so there is usually headroom to gain.
